I'm currently having a similar problem with my Xerox C20 black cartridge.  
It's a re-branded Lexmark Optra 45.    
 
One must eliminate the cartridge as a source of the problem,   
which is  _THE_ MOST_LIKELY_   cause.  The failure rate of   
new cartridges is only a few percent, but gets higher as the   
cartridges age.    
 
I've found the following (PitA) technique to work admirably.

Place the suspect cartridge in a coffee cup saucer with the   
nozzles in the center depression.   
 
Fill the depression with Windex, or another _AMMONIATED_ cleaner,   
and let it sit there for some tens of minutes.  Rinse off the   
cleaner and wipe nozzles dry before reinstalling.  You can do   
a crude readiness test by blotting the nozzles against a paper   
towel and observing the quality of the ink blot. I find I have   
to do this before every print job on two cartridges that I've   
refilled, probably with a less-than-optimum ink.  Oh well . . .

While the cartridge is out, I also take a moment to clean   
the gold-plated contacts with a Q-tip   
 
(very gently and carefully -- the gold is only a few microns thick!)
